1. Our Services

Tabernacle Animal Hospital provides veterinary examination, diagnostic, treatment, surgical, and related services for companion animals. Services are provided at the sole professional discretion of our licensed veterinarians based on each patient's individual medical needs.

2. Appointments & Scheduling

  • Appointments may be scheduled by phone, in person, or through online booking requests submitted via our website.
  • Online booking requests are subject to confirmation by our staff and are not considered final until confirmed.
  • We ask that you notify us as soon as possible if you need to reschedule or cancel an appointment.
  • Arriving significantly late for an appointment may require rescheduling, depending on the day's schedule.

By presenting your pet for examination or treatment, you (or your authorized agent) confirm that you are the owner or an authorized representative of the owner, and you consent to necessary examination and diagnostic procedures. Additional consent will be obtained prior to any significant treatment, procedure, surgery, or anesthesia, along with an estimate of associated costs when practical to provide.

4. Payment Terms

  • Payment for services is due at the time services are rendered, unless other arrangements have been made in advance with our office.
  • We accept major credit cards and other payment methods as posted at our hospital or on our website.
  • Estimates provided for treatment are good-faith approximations; actual costs may vary based on your pet's condition and response to treatment.
  • Accounts with an outstanding balance may be subject to collection efforts and associated fees as permitted by law.

5. Medical Records & Ownership

Medical records created by Tabernacle Animal Hospital are the property of the Hospital, maintained in accordance with New Jersey veterinary recordkeeping requirements. Copies of your pet's records will be provided to you or transferred to another veterinary provider upon request, as permitted by applicable law.

6. Limitation of Liability

While we are committed to providing high-quality veterinary care, the practice of veterinary medicine, like human medicine, cannot guarantee specific outcomes. To the fullest extent permitted by law, Tabernacle Animal Hospital's liability for any claim arising from services provided shall not exceed the amount paid for the specific service giving rise to the claim. We are not responsible for pre-existing conditions, complications inherent to medical or surgical treatment, or outcomes beyond our reasonable control.

7. Emergency & After-Hours Care

Please note: If your pet is experiencing a life-threatening emergency outside of our regular business hours, please contact your nearest 24-hour emergency veterinary hospital immediately. Tabernacle Animal Hospital's regular services are provided during normal business hours; after-hours emergency coverage details are provided directly by our office.

8. Client Conduct

We are committed to providing a safe and respectful environment for our clients, patients, and staff. We reserve the right to refuse or discontinue service to any client who engages in abusive, threatening, or unsafe behavior toward our team or other clients.

9. Website Use

Our website and any online forms or booking tools are provided for informational and scheduling convenience only. Information on our website does not constitute veterinary advice and is not a substitute for an in-person examination. In a medical emergency, please call our office or an emergency veterinary hospital directly rather than relying on website forms.

10. Governing Law

These Terms are governed by the laws of the State of New Jersey, without regard to its conflict of law principles. Any disputes arising from these Terms or our services shall be subject to the jurisdiction of the courts located in Burlington County, New Jersey.

11. Changes to These Terms

We may update these Terms from time to time to reflect changes in our practices or applicable law. The "Last Updated" date at the top of this page indicates when these Terms were most recently revised. Continued use of our services after changes are posted constitutes acceptance of the updated Terms.
